Process to prepare a project scope statement - how does it look like

In the Plan Scope Management Process we develop a Scope Management Plan, right?
And one of the topics in that Scope Management Plan should be the description of the process on how to prepare the detailed Project Scope Statement.

Can someone provide one (or more) examples of such a process description? I have done a little research already, but can't find much input for these. Real-Life examples preferred!

That would be the who, what, how, when, where, etc in preparation for writing the project scope statement. Who do you need to meet, what needs to be discussed, what needs to be included in the document, where to meet these people, when to meet them. It's the process of facilitation between people and events toward the production of a project scope statement.
