In a few words in your experience, why do you think some project managers failed or mishandle a project?

Many reasons like lack of experience, Not follow ethical and responsibilities tips, Not follow communication management procedures, lack of PM skills, Not all stakeholders were identified, lack of management knowledge plan, not follow information management system, not all requirements were collected and it's very important to create WBS and estimate cost and time to prepare baselines,...etc.
